Facilities will complete the forms and return them to ODEQ, where they will be reviewed for completeness. Problems and inconsistencies on applications and forms will be corrected by the applying facilities and ODEQ. Once complete and correct, applications will be forwarded to the SELSD for sample kit send out. Facilities will conduct sampling in accordance with the 3Ts guidance and the instructions provided by SELSD, then return the samples to the SELSD for analysis.
