Personally Identifiable Information

Per 74 O.S. SS 3106.4, the Oklahoma Department of Environmental Quality (DEQ) collects the following personally identifiable information for agency programs and some professional licenses and certifications: name, address, phone number, email address, employment information, Social Security numbers, birthdates, pictures, immigration documents, vehicle tag numbers, FEI numbers, tax information, Secretary of State filing numbers, insurance documents, drivers licenses, professional licenses, partial credit card numbers, and bank account numbers.

How is this information stored?

Personally Identifiable Information DEQ collects is stored electronically on servers maintained by the Office of Management and Enterprise Services, unless in hard copy format.  DEQ limits access to hard copies that contain personally identifiable information to the minimum necessary staff.

With whom does DEQ share personally identifiable information?

It is DEQ’s practice to keep personally identifiable information confidential, except as disclosure may be required by law or as otherwise authorized by the person to whom the personally identifiable information belongs.  DEQ protects all personal information in accordance with State of Oklahoma Information Security policies, procedures, guidelines (as applicable) found at the State of Oklahoma Information Security Policy, Information Security Policy, Procedures, Guidelines Procedures, Guidelines.
